Why use SBAR in a Disaster?
Disasters are chaotic. Providers are overwhelmed, multitasking, and under pressure.
You must lead the exchange—respectfully but clearly—to gather and give critical info.
SBAR = Structured, Fast, Familiar. Use it to cut through the noise and focus on action.
What’s “Modified” SBAR?
You're not just receiving info—you’re guiding the scene.
You’ll direct camera positioning, ask targeted questions, and interpret what you see.
The SBAR Breakdown
S – Situation
- What’s happening right now?
- Identify yourself and the patient (age, weight if possible).
- Ask: What are the immediate concerns?

B – Background
- What led to this situation? (e.g., bus crash, fire, collapse)
- What care has been given? Any test results or vitals?
Example: ....
A – Assessment
- What does the on-scene team think is critical?
- Do you agree based on what you see and hear?
Example: ....
R – Recommendation
- This is why they called you.
- Give a realistic, achievable recommendation based on current resources.
Example: At a rural site with blood products? Recommend weight-based transfusion for shock.
